  • Properly filled up SPES Application Form.
  • Parent Consent with the contact info of your parent or guardian (required for minors aged 15-17).
  • Photocopy of your Birth Certificate or Baptismal Certificate.
  • Copy of Form 137/138, Class Card, or your Report of Grades from the previous school year or semester.
  • Original Copy of the Barangay Certificate of Indigency (must be in the name of your parent or guardian).
  • Two pieces of passport-sized pictures.

For Out-of-School Youth:

  • Properly filled up SPES Application Form.

How to Apply for the Imus SPES Program

Applying for the Imus SPES Program involves a safe and contactless process. Just follow these six simple steps to secure your application:

  • Step 1: Fill Out Application Form. Scan the official QR code to download the application. Print it on Legal or Folio size bond paper.
  • Step 2: Fill Out Interview Questionnaire. Scan the second official QR code to answer the questionnaire to verify eligibility. Download and print this on Legal or Folio size bond paper.
  • Step 3: Submit Requirements. Place all necessary documents in a Long Brown Envelope. Make sure the application form, questionnaire, and all other required documents are complete.
  • Step 4: Drop Box Submission. Submit your sealed envelope through the designated drop box. The office practices a strict NO Contact Policy.
  • Step 5: Wait for Approval. The SPES slots remain strictly limited. Please patiently wait for your confirmation and slot availability.
  • Step 6: Deployment. If the office accepts you, they will assign you to work at a government office or a private company.
