DFA Appointment for Passport – Tagum City

The DFA operates a regional consular office situated on the 4th floor of Gaisano Mall of Tagum, which is the capital city of Davao Del Norte province. This office is designed to assist citizens in processing their passport applications, eliminating the need for them to travel to the nearby City of Davao.

TRANSACTION FEES

  • Regular Processing: PHP 950 pesos in 12 business days.
  • Expedited Processing: PHP 1,200 pesos in 7 business days.

Kindly be aware that an extra fee of Php 350.00 applies for Lost and Mutilated ePassports.

REQUIREMENTS

For Adult Applicants (18 years old and above)

  • Confirmed Online Appointment
  • Personal Appearance
  • Accomplished Application Form
  • A duly authenticated birth certificate from the Philippine Statistics Authority (PSA) on security paper, along with its photocopy, is required.
  • Married women using their spouse’s last name should also provide an original and a photocopy of a PSA Authenticated Marriage Contract on Security Paper or a Report of Marriage.
  • You will need to provide a Local Civil Registrar Copy if the PSA Birth Certificate is unclear or unreadable.
  • For RENEWAL of ePassport only – Current ePassport with a photocopy of the data page
  • For RENEWAL of non-ePassport only – Current Passport with photocopy of the data page
  • You can present any of the following valid identification cards along with one photocopy:
    • Philippine Identification (PhilID) / ePhilID
    • Social Security System (SSS) Card
    • Government Service Insurance System (GSIS) Card
    • Unified Multi-Purpose Identification (UMID) Card
    • Land Transportation Office (LTO) Driver’s License
    • Professional Regulatory Commission (PRC) ID
    • Overseas Workers Welfare Administration (OWWA) E-Card
    • Commission on Elections (COMELEC) Voter’s ID
    • Voter’s Certificate issued from COMELEC main office in Intramuros, Manila
    • Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
    • Senior Citizen ID
    • Airman License
    • Philippine Postal ID
    • Seafarer’s Record Book (SRB) or Seafarers Identity Document (SID)
    • School ID and Certificate of Registration

For MINOR applicants (below 18 years old)

  • Confirmed Online Appointment
  • Accomplished Application Form
  • Personal Appearance of minor applicant and either parent or authorized adult companion
  • A duly authenticated birth certificate from the Philippine Statistics Authority (PSA) on security paper, along with its photocopy, is required.
  • You will need to provide a Local Civil Registrar Copy if the PSA Birth Certificate is unclear or unreadable.
  • In the event that only one parent is accompanying the child, a Marriage Certificate must be provided.
  • In case the parents are unmarried and the mother is not accompanying the child, a Special Power of Attorney (SPA) executed by the mother will be necessary.
  • A Passport or a valid government-issued ID from either parent, along with one photocopy, is needed.
  • School ID and Certificate of Registration (if applicable)
  • For RENEWAL of ePassport only – Current ePassport with a photocopy of the data page
  • For RENEWAL of non-ePassport only – Current Passport with photocopy of the data page

PROCEDURE

To get a passport through this regional consular office, you need to do the following:

  1. (MANDATORY) Ensure that you establish a legitimate email address, preferably using Gmail or Yahoo.
  2. Visit DFA website at DFA Online Appointment System (passport.gov.ph/appointment) to schedule an appointment. When booking an appointment, ensure that you’ve furnished accurate information for a seamless transaction.
  3. Choose Start Individual Appointment.
  4. Choose TAGUM (GAISANO MALL OF TAGUM) as site.
  5. Select the date and time.
  6. Provide the following information: Personal Information, Family Information, Application Information, and Contact Information.
  7. Review and Confirm the provided information.
  8. Pay the corresponding fee by following the instructions provided in the email.
  9. Wait for the confirmation.
  10. Go to the consular office on the scheduled date and time with your requirements.
