Who Can Apply for QCID?

  • Residents of Quezon City
  • Persons with Disability (PWD)
  • Senior Citizens
  • Non-residents who are employed in Quezon City

Types of QCID

  • Resident QCID – for individuals living in Quezon City
  • PWD QCID – for persons with disability
  • Senior Citizen QCID
  • Non-Resident QCID – for those working but not residing in QC

Updating or Replacing Your QCID

Lost QCID Card

  • Affidavit of Loss
  • Photocopy of any valid ID

Defective QCID Card

  • Attestation Letter or Incident Report
  • Photocopy of any valid ID
  • Surrender old QCID card

Change of Address

  • Request Letter
  • Original Barangay Certificate of Residency (Standard Layout for QCID)
  • Photocopy of valid ID
  • Surrender old QCID card

Details Correction

  • Request Letter
  • Photocopy of valid ID
  • Surrender old QCID card
  • Supporting documents based on what detail needs to be corrected:
    • Name or Birthdate: PSA Birth Certificate
    • Signature: Valid ID with new signature or affidavit with old and new specimens
    • Blood Type: Medical Certificate or Affidavit
    • Civil Status:
      • Single to Married: Marriage Certificate
      • Married to Single: Annulment or Divorce Papers
      • Widow/Widower: Death Certificate of Spouse

Change of Email Address

  • Request Letter
  • Affidavit stating the old email is unrecoverable
  • Photocopy of valid ID and QCID card

QCID holders occasionally receive access to exclusive discounts. For instance, from time to time, mental health providers like the Psych Profile Center for Psychological Assessment Services offer limited-time discounts on services such as:

  • Counseling & Therapy

These offers are usually available by appointment, either online or in-clinic, and are subject to eligibility and clinic policies.
