A proper order form saves everyone time and prevents expensive mistakes. Here is the system we use with our regular team buyers.
What a Good Order Form Should Include
- Player information — Full name, jersey number, and position. Make sure numbers are unique for each player.
- Size selection — Dropdown or checkbox for S, M, L, XL, 2XL. Include a notes column for special sizing requests.
- Name on back — Spell every name twice. You would be surprised how often “Garcia” becomes “Garsia” in the final proof.
- Short preference — Same design as jersey or plain matching color. Specify length preference if offering short and long options.
- Quantity breakdown — Separate rows for player jerseys, coach/coaching staff, and extra stock.
- Deadline and delivery address — When do you need the jerseys and where should we send them?
Sample Format We Recommend
We ask our team buyers to submit a simple spreadsheet with these columns: No. | Player Name | Jersey Name | Number | Size | Shorts (Y/N). That is it. Five columns cover 90 percent of what we need. You can download and adapt it for your own procurement process.

Common Mistakes in Order Forms
- Duplicate numbers — always check that no two players choose the same number
- Missing sizes — make sure every name has a size selected
- Nicknames on back instead of official names — decide as a team what goes on the back
- No contact person listed — we always ask for a primary contact to avoid miscommunication
